Fostering Freedom Through Personalized Living Support

PCGH operates 6 adult care group homes or community housing programs spread throughout our community . These programs encourage clients to be an active part of their treatment team and be involved in planning their services.  Well trained Qualified Professionals and staff in these programs are available to assist with medical needs, securing services, scheduling and attending medical appointments and administering medications.  Direct Care staff are also available to work with clients on increasing their skills in the areas of personal care, daily living, finances, community access and social/leisure activities.  Direct Care staff are available on site while clients are at home, however, these homes do not have awake staff during 3^rd shift.  PCGH believes that community integration is an essential part of our client’s lives and provide opportunities for community activities and involvement.
