These community programs are for individuals with a diagnosis of IDD that need 24 hour support.  An interdisciplinary team (including a Registered Nurse, Registered Dietician, Psychologist, Speech Pathologist, Occupational Therapist, Qualified Professional, plus the guardian and individual served) are involved in the planning of the Individual Program Plan.  Well trained Direct Care staff are available on site 24 hours a day to provide active treatment while promoting independence.  Individuals receive training and support in the areas of personal care, daily living, finances, community access and social/leisure activities.  PCGH believes that community integration is an essential part of our client’s lives and provides opportunities for community activities and involvement.
